Application Form

Do not forget fill in all information and check off term of the examination in Prague!

The applicant will not be admitted to the entrance examination without the presented requirements from the list ABOVE.

  1. Application Form for Admission to study in English – GENERAL MEDICINE
  2. Verified copies of secondary/high school certificates and leaving certificate and official transcript, including all marks/grades and number of hours. This copy shoul be translated into English or Czech and must be certified with an Apostille by the Ministry of Foreign Affairs or Ministry of Education of the country you studied in.
  3. A Health Certificate from an authorized medical institution.
  4. One passport-size photograph.
  5. Photocopy of the 1st page of passport
  6. Confirmation of payment of the admission fee 35 Euro (non-refundable) to the faculty address (see above)
